## TL;DR

ADRIATIC brought the first improvement in curative-intent small-cell lung cancer treatment in 30 years: a year or two of immunotherapy after chemoradiation lengthens life.

## Summary

ADRIATIC, trial NCT03703297 sponsored by AstraZeneca and reported in 2024, brought the first improvement in curative-intent small-cell lung cancer treatment in thirty years: durvalumab consolidation for up to two years after concurrent chemoradiotherapy for limited-stage disease lengthens life. It randomised 730 patients without progression after chemoradiation to durvalumab or placebo and met both primary endpoints of overall and progression-free survival, leading to FDA approval on 4 December 2024, with benefit regardless of prophylactic cranial irradiation. OnCo links it to small-cell lung cancer, modern radiotherapy, prophylactic cranial irradiation, durvalumab, the limited-stage and PCI terms and Corinne Faivre-Finn, and it applies the PACIFIC template to small-cell disease. It is a single pivotal trial, and whether real-world consolidation series reproduce it is the open question.
