## TL;DR

In 2026 a dual-immunotherapy regimen plus lenvatinib added to TACE cut the risk of progression by about 30%; whether it extends life is not yet known.

## Summary

EMERALD-3, trial NCT05301842 sponsored by AstraZeneca and reported at ASCO 2026, showed that the dual immunotherapy STRIDE regimen of durvalumab and tremelimumab plus lenvatinib added to transarterial chemoembolisation cut the risk of progression by about thirty percent in embolisation-eligible unresectable hepatocellular carcinoma. It randomised 725 patients and met its primary progression-free survival endpoint at an interim analysis, with overall survival trending favourably but not formally tested, making it the third TACE combination trial positive for progression after EMERALD-1 and LEAP-012. OnCo links it to TACE, durvalumab, tremelimumab, lenvatinib and the TACE plus immunotherapy pairing. Whether it extends life, where LEAP-012 did not, is the question that will decide its place.
