## TL;DR

Many people can be cared for at the end of life in their own home, with visits from nurses, doctors and carers, equipment and medicines arranged by the hospice or community team. Ask early, because setting it up can take time.

## Summary

The NHS end of life care pages state that end of life care can be given at home, in hospital, in a care home or in a hospice, and cover planning, what to expect and the financial support available. Under Medicare, hospice care is usually given in the home or the facility where the person lives, and the plan of care can include nursing and medical services, aide and homemaker services, drugs and equipment for pain and symptom management, medical supplies, therapy services, social services, dietary counselling, short inpatient stays, and spiritual and grief counselling for the patient and family. In England, NHS continuing healthcare can fund care at home for people with complex needs, and its assessment routes include a Fast Track.
