If two drugs extend life equally but one makes patients much sicker, the health system should pay less for the sicker one. Build that into how prices are set.

## Summary

Health technology assessment uses quality-adjusted life-years in principle, but in oncology practice utilities are poorly measured and toxicity rarely moves the price.
