## TL;DR

## Summary

Orphan designation (FDA, EMA, MHLW, others) grants fee waivers, protocol assistance and exclusivity, and is applied for separately with different prevalence thresholds and evidence formats. FDA and EMA offer a common application form but decide independently. The proposal is reciprocal recognition: a designation from one stringent regulator is accepted by partners on notification, with local prevalence data required only where the condition's epidemiology plausibly differs.
