## Summary

iStopMM, trial NCT03327597 sponsored by the University of Iceland, screened an entire nation for monoclonal gammopathy of undetermined significance, the precursor of myeloma, to learn whether finding it early saves lives or just causes worry. About 75,000 Icelanders over forty took part, around 3,700 with MGUS were identified and randomised to no follow-up, standard follow-up or intensive follow-up, and early results between 2023 and 2025 show more smouldering myeloma and treatable lymphoproliferative disease detected in the intensive arm, with mortality outcomes pending. OnCo links it to multiple myeloma, the smouldering myeloma and MGUS term and Sigurdur Y. Kristinsson. Whether screening for a precursor condition improves survival or only diagnoses is the question it was designed to answer.
