## TL;DR

Showed that four cycles of cisplatin and vinorelbine after surgery for stage IB or II lung cancer lengthen life, and defined the regimen that became standard.

## Summary

JBR.10 randomised 482 patients with completely resected stage IB or II non-small-cell lung cancer and an ECOG performance status of 0 or 1 to vinorelbine plus cisplatin (242 patients) or observation (240). Forty-five percent had stage IB disease and 55 percent stage II; the median age was 61 and 53 percent had adenocarcinoma.

Adjuvant chemotherapy prolonged both disease-free and overall survival. The cost was real: neutropenia in 88 percent, including grade 3 febrile neutropenia in 7 percent, two deaths from toxic effects (0.8 percent), fatigue in 81 percent, nausea in 80 percent, anorexia in 55 percent and neuropathy in 48 percent, though severe non-haematological effects were uncommon.

JBR.10 is why cisplatin with vinorelbine, rather than cisplatin with etoposide, became the adjuvant backbone, and its long-term follow-up showed the benefit was confined to stage II disease, which is why stage IB is now a discussion rather than a recommendation.
