## TL;DR

Low-PV asked whether even low-risk patients gain from interferon: more of them stayed at the haematocrit target without progression when ropeginterferon was added to phlebotomy.

## Summary

Low-PV randomised 127 patients with low-risk PV (under 60 with no prior thrombosis) to phlebotomy plus low-dose aspirin with or without ropeginterferon alfa-2b. At 12 months, 84 percent on ropeginterferon kept a haematocrit at or below 45 percent without progressive disease versus 60 percent with phlebotomy alone (Barbui and colleagues, Lancet Haematology 2021), with fewer phlebotomies and no excess serious harm; the benefit held at two years.
