## TL;DR

In the US, Medicare can pay for a trained navigator to help people with serious illnesses including cancer coordinate care, find support services and speak up for themselves. Your doctor has to set it up at a visit, with your consent, and cost sharing may apply.

## Summary

Principal Illness Navigation (PIN) services are billed to Medicare with two HCPCS codes for services by certified or trained auxiliary personnel under the direction of a physician or other practitioner: G0023, PIN services including by a patient navigator, 60 minutes per calendar month, and G0024, each additional 30 minutes. The Rural Health Information Hub lists the services as person-centred planning, care coordination and health system navigation, referral to supportive and community-based resources, and promotion of patient self-advocacy skills, for serious high-risk conditions including cancer. An initiating visit (an evaluation and management visit, an annual wellness visit or certain behavioural assessments) establishes the need and a treatment plan; the practitioner must document consent before services start and annually, make sure the patient knows about cost sharing, and only one practitioner may bill the services per month.
