## TL;DR

The Milan System puts the result of a needle test on a salivary gland lump into one of six categories, each with its own risk of cancer and its own next step.

## Summary

The Milan System for Reporting Salivary Gland Cytopathology was first published in 2018. It defines six diagnostic categories for fine-needle aspiration covering non-neoplastic, benign and malignant lesions, and pairs each with a risk of malignancy and a clinical or surgical management algorithm. The second edition, July 2023 (Rossi and colleagues, Cytopathology), refines the risks from systematic reviews and meta-analyses, adds salivary gland imaging and ancillary testing, and updates nomenclature.
