## Summary

NEOGB (NCT06712420) randomises patients with locally advanced but resectable gallbladder carcinoma to neoadjuvant chemotherapy followed by curative surgery or to upfront surgery. Eligibility includes post-cholecystectomy gallbladder cancer with residual disease on imaging, a history of bile spillage or piecemeal removal during the index cholecystectomy, and regional lymph node involvement on imaging. The primary endpoint is improvement in three-year overall survival; 114 patients are planned at three Indian sites, recruitment began in January 2025 and primary completion is listed for April 2028. It complements POLCAGB (unresectable locally advanced disease) and OPT-IN (incidental T2 to T3 disease) as the third randomised test of neoadjuvant treatment in gallbladder cancer.
