## TL;DR

In England, adults with complex long-term health needs can have their care at home or in a care home arranged and paid for entirely by the NHS. A team of health professionals assesses whether you qualify; ask the hospital or community team to start an assessment.

## Summary

NHS continuing healthcare is free health and social care arranged and funded solely by the NHS for some people with long-term complex health needs. It can be given outside hospital, in the person's own home or in a care home. It is for adults; children and young people may receive a continuing care package instead. Eligibility is decided by a multidisciplinary team looking at what help is needed, how complex, intense and unpredictable the needs are. The NHS page names the assessment tools as the Checklist, the Decision Support Tool and the Fast Track.
