## TL;DR

O-RADS sorts a lump on or near an ovary seen on ultrasound or MRI into a numbered risk category, so the report says how likely it is to be cancer and what should happen next.

## Summary

O-RADS is the American College of Radiology's system for describing and classifying ovarian and adnexal lesions. It has two arms: ultrasound, the first test and tuned for sensitivity, and MRI, a second test that adds specificity. Both use a shared lexicon and assign a risk-of-malignancy category.

The ultrasound consensus guideline (Andreotti and colleagues, Radiology 2020) has six categories, O-RADS 0 to 5, covering the range from normal to high risk of malignancy, each with a management recommendation. It joins the North American pattern-based approach to the International Ovarian Tumor Analysis (IOTA) ADNEX risk model so that either route reaches the same category and the same advice.
