## TL;DR

The Japanese study in which the anti-CCR4 antibody mogamulizumab produced a response in half of people with relapsed adult T-cell leukaemia/lymphoma.

## Summary

A multicentre phase 2 study of KW-0761 (mogamulizumab), a humanised defucosylated anti-CCR4 antibody that enhances antibody-dependent cellular cytotoxicity, in relapsed aggressive CCR4-positive adult T-cell leukaemia-lymphoma. Patients received 1.0 mg/kg intravenously once a week for eight weeks. The primary endpoint was overall response; secondary endpoints were progression-free and overall survival from the first dose.

- Authors: Ishida T, Joh T, Uike N, et al.
- Findings: Of 28 patients enrolled, 27 received at least one infusion.; Objective responses were seen in 13 of 26 evaluable patients, including eight complete responses: overall response 50 per cent (95 per cent confidence interval 30 to 70).; Median progression-free survival was 5.2 months and median overall survival 13.7 months.; The commonest adverse events were infusion reactions (89 per cent) and skin rashes (63 per cent), manageable and reversible in all cases.
- What it means: The Japanese phase 2 evidence for mogamulizumab in relapsed disease; the 2019 international report records that the drug is approved in Japan for CCR4-positive adult T-cell leukaemia/lymphoma. The randomised study outside Japan later found a much lower confirmed response rate.
- Caveats: Single arm, 27 treated patients, all in Japan.; Responses were assessed by criteria specific to this disease, so the rate is not directly comparable with other lymphomas.
