## TL;DR

POP-RT, from Mumbai, showed that treating the pelvic lymph nodes as well as the prostate improves disease control in high-risk prostate cancer, reviving a question earlier trials had left unanswered.

## Summary

POP-RT randomised 224 men with high-risk, node-negative prostate cancer (most staged with PSMA PET) to image-guided prostate-only or whole-pelvic radiotherapy with two years of androgen deprivation. Five-year biochemical failure-free survival was 95 percent with pelvic radiotherapy and 81.2 percent with prostate-only treatment (hazard ratio 0.23), with better disease-free survival and more late urinary toxicity (Murthy and colleagues, Journal of Clinical Oncology 2021).
