## TL;DR

Adding the old blood-vessel antibody to a late-line chemotherapy pill extended survival by three months in patients who had exhausted standard options.

## Summary

SUNLIGHT, trial NCT04737187 sponsored by Taiho and Servier and reported in 2023, showed that adding the old anti-VEGF antibody bevacizumab to the late-line chemotherapy tablet trifluridine and tipiracil extends survival by a few months in refractory metastatic colorectal cancer. It randomised 492 patients and met its primary overall survival endpoint, leading to FDA approval in August 2023 and making the combination the preferred third-line option ahead of regorafenib or fruquintinib in many guidelines. OnCo links it to colorectal cancer, VEGF as a target, trifluridine and tipiracil, bevacizumab and Tae Won Kim, and the result is consistent with the Danish phase 2 trial. Whether the combination should be sequenced before or after fruquintinib, since the two have never been compared, is the open question.
