Mature five-year event-free survival was 79.6 percent with high-dose methotrexate against 75.2 percent with Capizzi methotrexate, with fewer marrow and central nervous system relapses and no increase in acute toxicity. Children aged 1 to 9 given dexamethasone and high-dose methotrexate did best, while patients aged 10 and over gained nothing from dexamethasone and had more osteonecrosis. High-dose methotrexate became the standard for high-risk childhood disease, which is how the corpus's high-risk childhood leukaemia page cites the trial.","status":"positive","asOf":"2026-09-22","links":[{"label":"ClinicalTrials.gov NCT00075725","url":"https://clinicaltrials.gov/study/NCT00075725"}],"tags":["soc-trials"],"related":[],"cancers":["all-paediatric-high-risk","all-leukemia","leukaemia"],"sections":[],"technologies":["cytotoxic-chemotherapy"],"targets":[],"drugs":["methotrexate","dexamethasone","prednisone","asparaginase"],"companies":[],"institutions":["childrens-oncology-group"],"pathways":[],"terms":[],"trials":[],"people":[],"bottlenecks":[],"keyPapers":["paper-aall0232-larsen-jco-2016"],"journals":[],"dependsOn":[],"notes":[],"nct":"NCT00075725","phase":"3","setting":"Newly diagnosed high-risk B-cell acute lymphoblastic leukaemia in patients aged 1 to 30: a 2 by 2 randomisation of dexamethasone or prednisone during induction and high-dose methotrexate or Capizzi escalating methotrexate with pegaspargase during interim maintenance","sponsor":"Children's Oncology Group","result":"Five-year event-free survival 79.6 percent with high-dose methotrexate against 75.2 percent with Capizzi methotrexate; dexamethasone benefited children aged 1 to 9 (five-year event-free survival 91.2 percent with high-dose methotrexate) but not older patients.","yearReported":2016,"enrolled":3154,"enrolledBasis":"registry","outcomes":[{"endpoint":"Event-free survival at 5 years, methotrexate randomisation (mature data)","primary":true,"unit":"%","arms":[{"name":"High-dose methotrexate with leucovorin rescue","value":79.6},{"name":"Capizzi escalating methotrexate with pegaspargase","value":75.2}],"p":"0.008","source":"https://doi.org/10.1200/JCO.2015.62.4544"},{"endpoint":"Event-free survival at 5 years, patients aged 1 to 9 by regimen","unit":"%","arms":[{"name":"Dexamethasone + high-dose methotrexate","value":91.2},{"name":"Dexamethasone + Capizzi methotrexate","value":83.2},{"name":"Prednisone + high-dose methotrexate","value":80.8},{"name":"Prednisone + Capizzi methotrexate","value":82.1}],"p":"0.015","source":"https://doi.org/10.1200/JCO.2015.62.4544"},{"endpoint":"Event-free survival at 5 years, patients aged 10 and over (registry results)","unit":"%","arms":[{"name":"Dexamethasone + high-dose methotrexate","value":77},{"name":"Dexamethasone + Capizzi methotrexate","value":69.1},{"name":"Prednisone + high-dose methotrexate","value":75.8},{"name":"Prednisone + Capizzi methotrexate","value":73.5}],"source":"https://clinicaltrials.gov/study/NCT00075725?tab=results"}]},"route":"/trials/aall0232/","neighbours":{"cancer":[{"id":"all-leukemia","kind":"cancer","name":"Acute lymphoblastic leukaemia","route":"/cancers/all-leukemia/"},{"id":"all-paediatric-high-risk","kind":"cancer","name":"High-risk acute lymphoblastic leukaemia in children (high-risk B-ALL and T-ALL)","route":"/cancers/all-paediatric-high-risk/"},{"id":"leukaemia","kind":"cancer","name":"Leukaemia (all types)","route":"/cancers/leukaemia/"}],"technology":[{"id":"cytotoxic-chemotherapy","kind":"technology","name":"Cytotoxic chemotherapy","route":"/technologies/cytotoxic-chemotherapy/"}],"drug":[{"id":"asparaginase","kind":"drug","name":"Asparaginase (pegaspargase, calaspargase pegol, Erwinia asparaginase)","route":"/drugs/asparaginase/"},{"id":"dexamethasone","kind":"drug","name":"Dexamethasone","route":"/drugs/dexamethasone/"},{"id":"methotrexate","kind":"drug","name":"Methotrexate","route":"/drugs/methotrexate/"},{"id":"prednisone","kind":"drug","name":"Prednisone","route":"/drugs/prednisone/"}],"institution":[{"id":"childrens-oncology-group","kind":"institution","name":"Children's Oncology Group (COG)","route":"/institutions/childrens-oncology-group/"}],"paper":[{"id":"paper-aall0232-larsen-jco-2016","kind":"paper","name":"AALL0232: dexamethasone and high-dose methotrexate improve outcome in high-risk B-cell acute lymphoblastic leukaemia in children and young adults","route":"/key-papers/paper-aall0232-larsen-jco-2016/"}]}}
