Children with completely resected, differentiated supratentorial tumours were observed; those with near-total or gross-total resection received immediate post-operative conformal radiotherapy to 59.4 Gy (54 Gy under 18 months); those with subtotal resection received chemotherapy (vincristine, carboplatin, cyclophosphamide, etoposide) before second-look surgery and radiotherapy. The primary endpoint was event-free survival.\n\nFive-year event-free survival was 68.5 percent after immediate conformal radiotherapy, 61.4 percent in the observation group and 37.2 percent after subtotal resection. Outcomes were similar for children under three, tumour grade mattered, and 1q gain in posterior fossa tumours predicted relapse (five-year event-free survival 47.4 against 82.8 percent). The corpus's ependymoma page describes maximal resection followed by conformal or proton radiotherapy to the tumour bed as the ACNS0121 approach.","status":"positive","asOf":"2026-09-22","links":[{"label":"ClinicalTrials.gov NCT00027846","url":"https://clinicaltrials.gov/study/NCT00027846"}],"tags":["soc-trials"],"related":[],"cancers":["ependymoma","brain-tumours","childhood-cancers"],"sections":[],"technologies":["imrt-igrt","cytotoxic-chemotherapy"],"targets":[],"drugs":["carboplatin","cyclophosphamide","etoposide","vincristine"],"companies":[],"institutions":["childrens-oncology-group"],"pathways":[],"terms":[],"trials":[],"people":[],"bottlenecks":[],"keyPapers":["paper-acns0121-ependymoma-conformal-radiotherapy-merchant-jco-2019"],"journals":[],"dependsOn":[],"notes":[],"nct":"NCT00027846","phase":"2","setting":"Newly diagnosed localised ependymoma in children aged 1 to 21: observation after complete resection of differentiated supratentorial tumours, immediate conformal radiotherapy after near-total or gross-total resection, and chemotherapy before second surgery and radiotherapy after subtotal resection","sponsor":"Children's Oncology Group","result":"Five-year event-free survival 68.5 percent with immediate post-operative conformal radiotherapy, 61.4 percent with observation after complete resection of differentiated supratentorial tumours, and 37.2 percent after subtotal resection with chemotherapy, second surgery and radiotherapy.","yearReported":2019,"enrolled":378,"enrolledBasis":"registry","outcomes":[{"endpoint":"Event-free survival at 5 years by treatment group","primary":true,"unit":"%","arms":[{"name":"Near-total or gross-total resection then immediate conformal radiotherapy","value":68.5,"note":"95% CI 62.8 to 74.2"},{"name":"Observation after complete resection of differentiated supratentorial ependymoma","value":61.4,"note":"95% CI 34.5 to 89.6"},{"name":"Subtotal resection: chemotherapy, second surgery, radiotherapy","value":37.2,"note":"95% CI 24.8 to 49.6"}],"source":"https://doi.org/10.1200/JCO.18.01765"},{"endpoint":"Event-free survival at 5 years, infratentorial tumours by 1q status","unit":"%","arms":[{"name":"Without 1q gain","value":82.8},{"name":"With 1q gain","value":47.4}],"p":"0.0013","source":"https://doi.org/10.1200/JCO.18.01765"}]},"route":"/trials/acns0121/","neighbours":{"cancer":[{"id":"brain-tumours","kind":"cancer","name":"Brain and spinal cord tumours (all types)","route":"/cancers/brain-tumours/"},{"id":"childhood-cancers","kind":"cancer","name":"Childhood cancers (all types)","route":"/cancers/childhood-cancers/"},{"id":"ependymoma","kind":"cancer","name":"Ependymoma","route":"/cancers/ependymoma/"}],"technology":[{"id":"cytotoxic-chemotherapy","kind":"technology","name":"Cytotoxic chemotherapy","route":"/technologies/cytotoxic-chemotherapy/"},{"id":"imrt-igrt","kind":"technology","name":"IMRT / IGRT (modern external beam)","route":"/technologies/imrt-igrt/"}],"drug":[{"id":"carboplatin","kind":"drug","name":"Carboplatin","route":"/drugs/carboplatin/"},{"id":"cyclophosphamide","kind":"drug","name":"Cyclophosphamide","route":"/drugs/cyclophosphamide/"},{"id":"etoposide","kind":"drug","name":"Etoposide","route":"/drugs/etoposide/"},{"id":"vincristine","kind":"drug","name":"Vincristine","route":"/drugs/vincristine/"}],"institution":[{"id":"childrens-oncology-group","kind":"institution","name":"Children's Oncology Group (COG)","route":"/institutions/childrens-oncology-group/"}],"paper":[{"id":"paper-acns0121-ependymoma-conformal-radiotherapy-merchant-jco-2019","kind":"paper","name":"ACNS0121: conformal radiotherapy for paediatric ependymoma, chemotherapy for incompletely resected tumours and observation after complete resection","route":"/key-papers/paper-acns0121-ependymoma-conformal-radiotherapy-merchant-jco-2019/"}]}}
