Because the liver removes most of the drug on first pass, hepatic arterial infusion through an implanted pump gives the liver metastases many times the exposure that systemic therapy achieves. Randomised trials, mostly from Memorial Sloan Kettering, showed higher hepatic response rates and, combined with systemic chemotherapy after liver resection, better hepatic disease-free survival.
