Testing matters because it changes surveillance, treatment and which relative can safely donate stem cells.","summary":"The WHO (2022) and the International Consensus Classification recognise myeloid neoplasms with germline predisposition. The AACR 2024 update groups the predispositions by the risk of MDS: without other organ dysfunction (DDX41, RUNX1, ANKRD26, ETV6), with organ dysfunction (GATA2 deficiency with at least 80% lifetime MDS/AML penetrance, SAMD9/SAMD9L), and bone marrow failure syndromes (Fanconi anaemia, Shwachman-Diamond syndrome, telomere biology disorders, severe congenital neutropenia, Diamond-Blackfan anaemia). CEBPA and PAX5 predispose to acute leukaemia without a risk of MDS.\n\nGeneral advice for all: at least annual examination, education about symptoms, care at a centre with expertise, early consultation with a transplant specialist, a baseline blood count and, for those at risk of MDS in childhood, a baseline marrow. For syndromes with prognostic data, serial somatic gene panels on marrow detect clonal evolution (TP53 in Shwachman-Diamond syndrome, monosomy 7 with SETBP1, ASXL1 or RUNX1 in GATA2 deficiency). NCCN recommends predisposition testing in all patients with MDS and adults under 50 with AML; related donors are tested.","asOf":"2026-11-04","links":[{"label":"Maese, Clin Cancer Res 2024: surveillance for children with predisposition to haematopoietic malignancy (AACR update)","url":"https://doi.org/10.1158/1078-0432.ccr-24-0685"},{"label":"NHS England National Genomic Test Directory, rare and inherited disease, version 9 (spreadsheet)","url":"https://www.england.nhs.uk/wp-content/uploads/2018/08/rare-and-inherited-disease-national-genomics-test-directory-v9.xlsx"}],"tags":["gap-fill","hereditary"],"related":["ddx41-myeloid-predisposition","familial-platelet-disorder-runx1","fanconi-anaemia","shwachman-diamond-syndrome","dyskeratosis-congenita"],"cancers":["aml","mds"],"sections":[],"technologies":["allogeneic-hsct","germline-testing"],"targets":["ddx41","runx1","gata2","cebpa","etv6"],"drugs":[],"companies":[],"institutions":[],"pathways":[],"terms":["hereditary-cancer-syndromes"],"trials":[],"people":[],"bottlenecks":[],"keyPapers":[],"journals":[],"dependsOn":[],"notes":["NHS England genomic test directory: R347, inherited predisposition to acute myeloid leukaemia (AML)."],"category":"Genomics & genetics"},"route":"/terms/germline-myeloid-predisposition/","neighbours":{"term":[{"id":"ddx41-myeloid-predisposition","kind":"term","name":"DDX41-associated predisposition to myeloid neoplasms","route":"/terms/ddx41-myeloid-predisposition/"},{"id":"dyskeratosis-congenita","kind":"term","name":"Dyskeratosis congenita and telomere biology disorders","route":"/terms/dyskeratosis-congenita/"},{"id":"fanconi-anaemia","kind":"term","name":"Fanconi anaemia","route":"/terms/fanconi-anaemia/"},{"id":"hereditary-cancer-syndromes","kind":"term","name":"Hereditary cancer syndromes","route":"/terms/hereditary-cancer-syndromes/"},{"id":"familial-platelet-disorder-runx1","kind":"term","name":"RUNX1 familial platelet disorder with associated myeloid malignancy","route":"/terms/familial-platelet-disorder-runx1/"},{"id":"shwachman-diamond-syndrome","kind":"term","name":"Shwachman-Diamond syndrome","route":"/terms/shwachman-diamond-syndrome/"}],"cancer":[{"id":"aml","kind":"cancer","name":"Acute myeloid leukaemia","route":"/cancers/aml/"},{"id":"mds","kind":"cancer","name":"Myelodysplastic syndromes / neoplasms (MDS)","route":"/cancers/mds/"}],"technology":[{"id":"allogeneic-hsct","kind":"technology","name":"Allogeneic stem cell transplantation","route":"/technologies/allogeneic-hsct/"},{"id":"germline-testing","kind":"technology","name":"Germline (hereditary) testing","route":"/technologies/germline-testing/"}],"target":[{"id":"cebpa","kind":"target","name":"CEBPA","route":"/targets/cebpa/"},{"id":"ddx41","kind":"target","name":"DDX41","route":"/targets/ddx41/"},{"id":"etv6","kind":"target","name":"ETV6","route":"/targets/etv6/"},{"id":"gata2","kind":"target","name":"GATA2","route":"/targets/gata2/"},{"id":"runx1","kind":"target","name":"RUNX1","route":"/targets/runx1/"}],"drug":[{"id":"ambry-cancernext","kind":"drug","name":"CancerNext and CancerNext-Expanded (Ambry Genetics)","route":"/drugs/ambry-cancernext/"}]}}
