Sixteen randomised trials in bowel surgery found 40 per cent fewer complications and stays more than two days shorter, but in 151 American hospitals given a full implementation programme, most failed to improve compliance with its elements.","summary":"What it is. Enhanced recovery after surgery is a bundle: preparation and information before the operation, a carbohydrate drink rather than prolonged fasting, restricted intravenous fluids, avoiding routine drains and tubes, multimodal pain control that limits opioids, and eating and walking early afterwards.\n\nThe evidence. A meta-analysis of 16 randomised trials with 2,376 patients having colorectal surgery found the pathway reduced overall complications (relative risk 0.60, 95 per cent confidence interval 0.46 to 0.76), mostly non-surgical ones such as chest infections, and shortened hospital stay by 2.28 days without increasing readmissions. In the international ERAS registry of 2,352 colorectal cancer resections in 13 centres, increasing compliance with the protocol was independently associated with fewer complications and shorter admission; restrictive fluids were associated with fewer complications, and carbohydrate loading with shorter stay.\n\nThe measured gap. A 2023 study of 151 United States hospitals that each implemented a colorectal pathway with an 18-month programme of materials, education and data capture measured six process elements: of 663 opportunities to improve, 20 per cent achieved a substantial improvement, 52 per cent minimal improvement and 28 per cent got worse. A single-institution study of 1,519 colorectal operations found that patients of low socioeconomic status had lower completion of bowel preparation and carbohydrate loading and longer stays. A pathway on paper is not a pathway delivered.\n\nThe upgrade, then, is not to adopt enhanced recovery, which most surgical units say they have, but to measure each element for each patient and feed the figures back. The corpus record `eras-perioperative-nutrition` describes the pathway itself.","asOf":"2026-10-20","links":[{"label":"Greco et al., Enhanced recovery program in colorectal surgery: a meta-analysis of randomized controlled trials (World Journal of Surgery 2014)","url":"https://doi.org/10.1007/s00268-013-2416-8"},{"label":"ERAS Compliance Group, The impact of enhanced recovery protocol compliance on elective colorectal cancer resection: results from an international registry (Annals of Surgery 2015)","url":"https://doi.org/10.1097/SLA.0000000000001029"},{"label":"Shah et al., Truth of colorectal enhanced recovery programs: process measure compliance in 151 hospitals (Journal of the American College of Surgeons 2023)","url":"https://doi.org/10.1097/xcs.0000000000000562"},{"label":"Antoniv et al., Enhanced recovery after surgery protocol compliance and early outcomes for elective colorectal procedures by race and ethnicity and socioeconomic status (Annals of Surgery 2026)","url":"https://doi.org/10.1097/sla.0000000000006622"}],"tags":["protocol-upgrade","upgrade:proven-unevenly-used","evidence:strong"],"related":["idea-pu-shorter-fasting-before-surgery","idea-pu-extended-clot-prevention-after-surgery","idea-bio2-prehabilitation-standard"],"cancers":["colorectal","rectal-cancer","gastric","esophageal","pancreatic"],"sections":["surgery"],"technologies":["eras-perioperative-nutrition","prehabilitation","oncology-nutrition"],"targets":[],"drugs":[],"companies":[],"institutions":[],"pathways":[],"terms":[],"trials":[],"people":[],"bottlenecks":["b-knowledge-diffusion","b-care-fragmentation","b-surgery-radiation-innovation"],"keyPapers":[],"journals":[],"dependsOn":[],"notes":[],"hypothesis":"Routine per-patient measurement of enhanced recovery elements, with monthly feedback to surgical teams, raises compliance above 80 per cent within a year and reduces complications and length of stay after cancer surgery, with the largest gain in patients from deprived areas.","rationale":"The randomised evidence for the bundle is strong, compliance predicts outcome in the registry, and the largest implementation study measured shows that most hospitals do not move compliance without something more than materials and education.","test":"A stepped-wedge trial of compliance measurement and feedback across surgical units, with compliance per element, complications, length of stay and readmissions as endpoints, analysed by deprivation quintile.","maturity":"being-tested-at-scale","actor":"clinic","cost":"small","horizonYears":2},"route":"/ideas/idea-pu-enhanced-recovery-with-measured-compliance/","neighbours":{"idea":[{"id":"idea-pu-shorter-fasting-before-surgery","kind":"idea","name":"Clear drinks until two hours before an operation, rather than nothing from midnight","route":"/ideas/idea-pu-shorter-fasting-before-surgery/"},{"id":"idea-pu-extended-clot-prevention-after-surgery","kind":"idea","name":"Four weeks of blood-clot prevention after major abdominal or pelvic cancer surgery","route":"/ideas/idea-pu-extended-clot-prevention-after-surgery/"},{"id":"idea-bio2-prehabilitation-standard","kind":"idea","name":"Four weeks of training and nutrition before major cancer surgery, as standard","route":"/ideas/idea-bio2-prehabilitation-standard/"}],"cancer":[{"id":"colorectal","kind":"cancer","name":"Colorectal cancer","route":"/cancers/colorectal/"},{"id":"gastric","kind":"cancer","name":"Gastric & gastro-oesophageal junction cancer","route":"/cancers/gastric/"},{"id":"esophageal","kind":"cancer","name":"Oesophageal cancer","route":"/cancers/esophageal/"},{"id":"pancreatic","kind":"cancer","name":"Pancreatic ductal adenocarcinoma","route":"/cancers/pancreatic/"},{"id":"rectal-cancer","kind":"cancer","name":"Rectal cancer","route":"/cancers/rectal-cancer/"}],"section":[{"id":"surgery","kind":"section","name":"Surgery & Interventional","route":"/fronts/surgery/"}],"technology":[{"id":"eras-perioperative-nutrition","kind":"technology","name":"Enhanced recovery (ERAS) and perioperative nutrition","route":"/technologies/eras-perioperative-nutrition/"},{"id":"oncology-nutrition","kind":"technology","name":"Nutrition support and cachexia management","route":"/technologies/oncology-nutrition/"},{"id":"prehabilitation","kind":"technology","name":"Prehabilitation before cancer surgery","route":"/technologies/prehabilitation/"}],"bottleneck":[{"id":"b-care-fragmentation","kind":"bottleneck","name":"Fragmented care and guideline gaps","route":"/bottlenecks/b-care-fragmentation/"},{"id":"b-knowledge-diffusion","kind":"bottleneck","name":"Knowledge reaches practice too slowly","route":"/bottlenecks/b-knowledge-diffusion/"},{"id":"b-surgery-radiation-innovation","kind":"bottleneck","name":"Surgery and radiotherapy cure most, get least","route":"/bottlenecks/b-surgery-radiation-innovation/"}]}}
