A randomised trial of 208 people in India found that music and a guided relaxation exercise during the first five sessions lowered mask anxiety scores.","summary":"How common. In one cohort of 100 people having head and neck radiotherapy, 26 per cent identified themselves as anxious about the mask on a modified distress thermometer. In a second study of 65 people, 43 per cent had clinically significant anxiety before the mask-making session and 30 to 43 per cent across trajectory groups before treatments; the radiographers' ratings of each patient's anxiety agreed poorly with the patient's own, so asking works better than watching. In a cohort of 35 people with mask anxiety followed through treatment, it fell over time for 72 per cent and stayed constant for 22 per cent.\n\nWhat helps. In a randomised trial at a hospital in central India, 216 people were assigned to pre-recorded instrumental music with a guided relaxation and visualisation exercise during their first five sessions, or to standard care; among 208 analysed, median mask-anxiety scores after five sessions were 2 against 6 (p below 0.001), with no adverse events and four people stopping because they found it distracting. Patients followed in the 35-person cohort reported relying on staff, self-taught strategies, music, visualisation and medication.\n\nThe upgrade has two parts that cost almost nothing: a one-question screen at mask-making and at intervals, because anxiety changes over the course and staff cannot see it, and an offer of music and a recorded relaxation exercise to anyone who screens positive. Adoption of routine screening is not measured by any source OnCo found.","asOf":"2026-10-20","links":[{"label":"Nixon et al., Exploring the prevalence and experience of mask anxiety for the person with head and neck cancer undergoing radiotherapy (Journal of Medical Radiation Sciences 2018)","url":"https://doi.org/10.1002/jmrs.308"},{"label":"Nixon et al., A prospective examination of mask anxiety during radiotherapy for head and neck cancer and patient perceptions of management strategies (Journal of Medical Radiation Sciences 2019)","url":"https://doi.org/10.1002/jmrs.346"},{"label":"Burns et al., Trajectory of anxiety related to radiation therapy mask immobilization and treatment delivery in head and neck cancer, and radiation therapists' ability to detect this anxiety (Advances in Radiation Oncology 2022)","url":"https://doi.org/10.1016/j.adro.2022.100967"},{"label":"Joshi et al., Effect of music therapy and relaxation visualization intervention on mask anxiety in head and neck cancer patients undergoing radiotherapy: a randomized controlled trial (Supportive Care in Cancer 2025)","url":"https://doi.org/10.1007/s00520-025-09901-z"}],"tags":["protocol-upgrade","upgrade:promising","evidence:moderate"],"related":["idea-pu-open-face-masks","idea-pu-radiotherapy-walkthrough-before-planning"],"cancers":["head-and-neck","oropharyngeal-cancer"],"sections":["radiation"],"technologies":["music-therapy-cancer","relaxation-guided-imagery","rejuv-mind-distress-screening","psycho-oncology"],"targets":[],"drugs":[],"companies":[],"institutions":[],"pathways":[],"terms":[],"trials":[],"people":[],"bottlenecks":["b-toxicity-qol","b-patient-voice"],"keyPapers":[],"journals":[],"dependsOn":[],"notes":[],"hypothesis":"Routine screening for mask anxiety at mask-making and weekly, with music and guided relaxation offered to those who screen positive, reduces anxiety scores, use of sedatives and missed or delayed sessions over a course of head and neck radiotherapy.","rationale":"Prevalence is measured, staff detection is measured to be poor, and one adequately sized randomised trial found a cheap intervention worked over the first week.","test":"A cluster-randomised trial across radiotherapy departments of routine screening plus a music and relaxation offer against usual care, with anxiety scores, sedative use and treatment interruptions as endpoints, run outside India to test whether the effect transfers.","maturity":"early-clinical","actor":"clinic","cost":"small","horizonYears":2},"route":"/ideas/idea-pu-mask-anxiety-screening/","neighbours":{"idea":[{"id":"idea-pu-open-face-masks","kind":"idea","name":"Open-face immobilisation masks for brain and head and neck radiotherapy where surface guidance can check the position","route":"/ideas/idea-pu-open-face-masks/"},{"id":"idea-pu-radiotherapy-walkthrough-before-planning","kind":"idea","name":"Show people the radiotherapy room and the treatment before the planning appointment, using virtual reality or a guided visit","route":"/ideas/idea-pu-radiotherapy-walkthrough-before-planning/"}],"cancer":[{"id":"head-and-neck","kind":"cancer","name":"Head and neck squamous cell carcinoma","route":"/cancers/head-and-neck/"},{"id":"oropharyngeal-cancer","kind":"cancer","name":"Oropharyngeal cancer (tonsil and base of tongue)","route":"/cancers/oropharyngeal-cancer/"}],"section":[{"id":"radiation","kind":"section","name":"Radiation Therapy","route":"/fronts/radiation/"}],"technology":[{"id":"music-therapy-cancer","kind":"technology","name":"Music therapy and music medicine","route":"/technologies/music-therapy-cancer/"},{"id":"psycho-oncology","kind":"technology","name":"Psycho-oncology and distress screening","route":"/technologies/psycho-oncology/"},{"id":"relaxation-guided-imagery","kind":"technology","name":"Relaxation training and guided imagery","route":"/technologies/relaxation-guided-imagery/"},{"id":"rejuv-mind-distress-screening","kind":"technology","name":"Screening for distress: what the thermometer can and cannot do","route":"/technologies/rejuv-mind-distress-screening/"}],"bottleneck":[{"id":"b-patient-voice","kind":"bottleneck","name":"Patients lack understanding, navigation and agency","route":"/bottlenecks/b-patient-voice/"},{"id":"b-toxicity-qol","kind":"bottleneck","name":"Toxicity and quality of life are undervalued","route":"/bottlenecks/b-toxicity-qol/"}]}}
