{"entity":{"id":"panfire-2","kind":"trial","name":"PANFIRE-2","aka":["PANFIRE 2","PANFIRE-II"],"tldr":"PANFIRE-2 treated 50 people with electric-pulse ablation of pancreatic tumours that could not be removed; they lived a median 17 months from diagnosis, longer than the trial's historical target, but more than half had complications and nearly half relapsed locally, so the treatment stays in specialist centres and trials.","summary":"Fifty participants (median age 61) were treated between December 2012 and September 2017. Median overall survival was 17 months from diagnosis of locally advanced disease (95 percent confidence interval 15 to 19) and 10 months from electroporation, the same 17 months whether patients had received FOLFIRINOX (22) or gemcitabine-based or no induction (18); for local recurrence it was 16 months from recurrence. Local recurrence after electroporation developed in 23 of 50 (46 percent); tumour volume of 37 cm3 or more, CA 19-9 of 2,000 U/mL or more and a fall of 50 percent or less after ablation predicted worse survival. There were 14 minor and 21 major complications in 29 participants (58 percent) and two deaths within 90 days, one probably related. The earlier phase 1/2 PANFIRE (25 patients) had reported 11 months from ablation and major complications in 10 patients. CROSSFIRE then found electroporation equivalent to stereotactic radiotherapy; NCCN lists irreversible electroporation as an option in experienced centres and the UK LAP-PIE study tested feasibility of a trial after chemotherapy.","status":"completed","asOf":"2026-09-24","links":[{"label":"ClinicalTrials.gov NCT01939665","url":"https://clinicaltrials.gov/study/NCT01939665"},{"label":"Ruarus et al., PANFIRE-2 (Radiology 2020)","url":"https://doi.org/10.1148/radiol.2019191109"},{"label":"Scheffer et al., PANFIRE phase 1/2 (Radiology 2017)","url":"https://doi.org/10.1148/radiol.2016152835"}],"tags":[],"related":[],"cancers":["pancreatic","locally-advanced-pdac"],"sections":[],"technologies":["irreversible-electroporation"],"targets":[],"drugs":[],"companies":[],"institutions":[],"pathways":[],"terms":["ca19-9"],"trials":["crossfire"],"people":[],"bottlenecks":[],"keyPapers":[],"journals":[],"dependsOn":[],"notes":[],"nct":"NCT01939665","phase":"2","setting":"Locally advanced pancreatic cancer (40 patients) or local recurrence after resection (10) at Dutch centres: percutaneous CT-guided irreversible electroporation after induction chemotherapy where given, a single-arm phase 2 with median overall survival from diagnosis as the primary endpoint","sponsor":"Amsterdam University Medical Centre","result":"Median overall survival 17 months from diagnosis (95 percent confidence interval 15 to 19) and 10 months from electroporation in locally advanced disease; local recurrence 46 percent; major complications in 21 events across 29 of 50 participants.","yearReported":2020,"enrolled":50,"enrolledBasis":"registry","outcomes":[{"endpoint":"Overall survival from diagnosis, locally advanced cohort","primary":true,"unit":"months","arms":[{"name":"Percutaneous irreversible electroporation","n":40,"value":17,"note":"95% CI 15 to 19; target 11.6 to 14.9 months"}],"source":"https://doi.org/10.1148/radiol.2019191109"}]},"route":"/trials/panfire-2/","neighbours":{"cancer":[{"id":"locally-advanced-pdac","kind":"cancer","name":"Locally advanced unresectable pancreatic ductal adenocarcinoma","route":"/cancers/locally-advanced-pdac/"},{"id":"pancreatic","kind":"cancer","name":"Pancreatic ductal adenocarcinoma","route":"/cancers/pancreatic/"}],"technology":[{"id":"irreversible-electroporation","kind":"technology","name":"Irreversible electroporation (NanoKnife)","route":"/technologies/irreversible-electroporation/"}],"term":[{"id":"ca19-9","kind":"term","name":"CA 19-9","route":"/terms/ca19-9/"}],"trial":[{"id":"crossfire","kind":"trial","name":"CROSSFIRE","route":"/trials/crossfire/"}]}}
