LATITUDE: abiraterone plus prednisone in newly diagnosed high-risk metastatic castration-sensitive prostate cancer
Adding abiraterone to hormone therapy at the time of diagnosis of high-risk metastatic prostate cancer cut deaths by more than a third, establishing early intensification instead of waiting for castration resistance.
Overview
Phase 3 placebo-controlled trial of 1,199 men with newly diagnosed high-risk metastatic castration-sensitive prostate cancer (at least two of Gleason 8 or more, three or more bone lesions, visceral metastases) randomised to androgen deprivation with abiraterone plus prednisone or placebo.
At the first analysis overall survival at three years was 66 versus 49 percent (hazard ratio 0.62) and radiographic progression-free survival 33.0 versus 14.8 months; the final analysis showed median survival of 53.3 versus 36.5 months.
- Median overall survival 53.3 vs 36.5 months at final analysis; hazard ratio 0.66.
- Radiographic progression-free survival 33.0 vs 14.8 months; hazard ratio 0.47.
Abiraterone with androgen deprivation is a standard first-line doublet for metastatic hormone-sensitive prostate cancer, consistent with the STAMPEDE result.
- Restricted to high-risk disease; STAMPEDE showed benefit across risk groups.
- Hypertension and hypokalaemia require monitoring.
