MAPK inhibitor redifferentiation → radioiodine
A short course of a MEK or BRAF pill can coax iodine-resistant thyroid cancer into taking up iodine again, letting radioactive iodine work once more.
Selumetinib (Ho et al., NEJM 2013), dabrafenib, and trametinib restore radioiodine uptake in roughly half of RAI-refractory patients, especially RAS-mutant; durable responses follow in some. The adjuvant version (ASTRA) failed, so the approach is for refractory disease only.
