Intrahepatic, perihilar, distal and gallbladder cancer
Bile duct cancers are named by where they start: inside the liver, at the hilum where the ducts join, in the lower duct near the pancreas, or in the gallbladder. Each behaves and mutates differently.
Intrahepatic cholangiocarcinoma (iCCA) carries FGFR2 fusions (~10-15%) and IDH1 mutations (~15%) and is rising in incidence; perihilar (Klatskin) and distal tumours are more often HER2-amplified or KRAS-mutant; gallbladder cancer has the highest HER2 prevalence (~15-20%) and is common in Chile, India and among Indigenous Americans. Molecular profiling is guideline-recommended for all advanced biliary cancers.
