Bile duct cancer inside the liver is staged by the number of tumours, their size (5 cm is the cut-off) and whether they invade blood vessels, then by spread to nodes and distant organs.
AJCC chapter 23 covers the intrahepatic bile ducts and is an 8th edition chapter in the AJCC 2026 table. In the 8th edition tables reproduced by the NCI, stage 0 is carcinoma in situ inside the duct; stage IA is a single tumour of 5 cm or less without vascular invasion and IB a single tumour larger than 5 cm without it; stage II is a single tumour with vascular invasion inside the liver, or several tumours. Perihilar and distal bile duct cancers have their own chapters (25 and 26).
Most pancreatic cancers arise in the head next to the bile duct, which is why jaundice is the presenting sign; bile duct cancers are named by where along the tree they sit.
