Since 2023 the stage of womb cancer includes its histological type and, where tested, its molecular class, so two tumours of the same size can now carry different stages.
FIGO's 2023 revision (Berek and colleagues, International Journal of Gynecology and Obstetrics 2023) replaced the 2009 staging. Stage I is split into IA1 to IA3, IB and IC by histological type, depth of invasion into the muscle and lymphovascular space invasion, with aggressive types (serous, high-grade endometrioid, clear cell, carcinosarcoma and others) moved to IC or IIC; stage II separates cervical stromal invasion, substantial lymphovascular invasion and aggressive types with any muscle invasion; stage III and IV substages separate adnexal from serosal spread, pelvic from para-aortic nodes, and pelvic from extrapelvic peritoneal and distant spread. Molecular classification (POLEmut, MMRd, NSMP, p53abn) is encouraged for all; when known it is added to the stage (for example IAmPOLEmut or IICmp53abn), and in stages I and II p53abn or POLEmut moves the stage up or down.
Most high-grade ovarian cancers begin at the tip of the fallopian tube; endometrial cancer lines the uterus, cervical cancer starts at the transformation zone; each drains to a different node group.
