Bone cancers have an AJCC staging chapter, but in practice osteosarcoma is usually grouped simply as localised or metastatic, because that is what changes treatment.
AJCC chapter 38 covers primary bone tumours and is an 8th edition chapter in the AJCC 2026 table. The NCI's patient summary for osteosarcoma states that instead of being grouped by stage, osteosarcoma and undifferentiated pleomorphic sarcoma of bone are grouped as localised or metastatic. Bone sarcomas favour the fast-growing ends of long bones (osteosarcoma) or the shaft (Ewing), soft tissue sarcomas the deep muscle compartments; spread is through the blood to the lungs, rarely via lymph nodes.
Lymph node spread is rare (except epithelioid, synovial, clear cell and rhabdomyosarcoma); sarcomas go through the blood to the lungs.
