Five Wishes is a plain-language US advance directive in which you name who decides for you and write down the medical care, comfort and spiritual support you want, and what you want your family to know.
Five Wishes is a programme of the US nonprofit Aging with Dignity, which describes it as the most widely used advance directive in America. The document has five parts: a health care power of attorney (who can make decisions for me), a living will (the kind of medical treatment I want or don't want), physical comfort, spiritual comfort, and what I want my loved ones to know. Its site states that it meets legal requirements in 46 states and is available on paper or completed and signed online.
