The Serious Illness Conversation Guide is a set of questions clinicians use to ask people with serious illness what matters most to them, so that their care can follow it. You can ask your team to have this conversation with you.
The Serious Illness Conversation Guide was developed by palliative care experts at Ariadne Labs as part of the Serious Illness Care Program, a system-level care delivery model. Ariadne Labs describes the guide as a framework for physicians, nurses, social workers, chaplains and other clinicians to explore what is most important to patients, and offers it for download with clinician training and support for health systems. The programme also publishes resources for patients and families on sharing wishes.
