imRECIST is a variant of the scan-measuring rules from atezolizumab trials that lets a response after apparent growth count, to capture benefit from immunotherapy that standard rules miss.
Hodi and colleagues (Journal of Clinical Oncology 2018) applied immune-modified RECIST to atezolizumab trial data in lung, bladder and kidney cancer and melanoma. It allows a best response after progressive disease and changes how new and non-target lesions define progression. Compared with RECIST 1.1, best overall response was 1 to 2 percentage points higher, disease control 8 to 13 points higher and median progression-free survival 0.5 to 1.5 months longer. Progression defined only by new lesions was not associated with longer survival.
