MYCN amplification
Extra copies of the MYCN oncogene, found in about 20% of neuroblastomas, mark the most aggressive disease and define high risk at any age.
Detected by FISH (>10 copies); associated with rapid progression, 1p deletion and poor outcome. MYCN is not directly druggable; approaches include BET/Aurora A inhibitors (indirect), ODC1/polyamine inhibition (eflornithine is a MYCN-pathway drug), and ALK co-mutation (lorlatinib in ANBL1531).
