COG ANBL1531

COG ANBL1531 is the current North American high-risk trial, adding targeted radiation during induction and an ALK pill for children whose tumours carry ALK mutations.

Also tests reduced-intensity therapy in the standard arm and eflornithine maintenance uptake. Primary MIBG randomisation results expected 2026-27; ALK arm (lorlatinib) reports encouraging response rates.

Newly diagnosed high-risk neuroblastoma: 131I-MIBG added to induction (randomised, MIBG-avid); lorlatinib added for ALK-aberrant tumours (non-randomised arm)
