PANFIRE-2 treated 50 people with electric-pulse ablation of pancreatic tumours that could not be removed; they lived a median 17 months from diagnosis, longer than the trial's historical target, but more than half had complications and nearly half relapsed locally, so the treatment stays in specialist centres and trials.
Fifty participants (median age 61) were treated between December 2012 and September 2017. Median overall survival was 17 months from diagnosis of locally advanced disease (95 percent confidence interval 15 to 19) and 10 months from electroporation, the same 17 months whether patients had received FOLFIRINOX (22) or gemcitabine-based or no induction (18); for local recurrence it was 16 months from recurrence. Local recurrence after electroporation developed in 23 of 50 (46 percent); tumour volume of 37 cm3 or more, CA 19-9 of 2,000 U/mL or more and a fall of 50 percent or less after ablation predicted worse survival. There were 14 minor and 21 major complications in 29 participants (58 percent) and two deaths within 90 days, one probably related. The earlier phase 1/2 PANFIRE (25 patients) had reported 11 months from ablation and major complications in 10 patients. CROSSFIRE then found electroporation equivalent to stereotactic radiotherapy; NCCN lists irreversible electroporation as an option in experienced centres and the UK LAP-PIE study tested feasibility of a trial after chemotherapy.
