Venetoclax + hypomethylating agent

Azacitidine primes leukaemia cells for death; venetoclax removes the BCL-2 shield. Together they roughly doubled remissions in older AML.

VIALE-A: OS 14.7 vs 9.6 months, CR 36.7% vs 17.9%. Now available fully orally (Inqovi + venetoclax, 2026). Duration of venetoclax (14 vs 28 days) is being shortened to reduce cytopenias without losing efficacy.

Rationale
Azacitidine lowers MCL-1 and induces NOXA, increasing dependence on BCL-2, which venetoclax then blocks.
