HER2 PET

HER2 PET is a PET scan using radiolabelled trastuzumab or smaller HER2 binders to map HER2 across all metastases at once.

89Zr-trastuzumab and 68Ga/18F-labelled HER2 affibodies and nanobodies (e.g., 68Ga-ABY-025, 18F-GE-226) have been studied in the ZEPHIR and IMPACT trials. Predictive of T-DM1 response and useful for heterogeneous HER2-low disease where a single biopsy is unrepresentative. Not yet approved.

Detector ring · ⁸⁹Zr-trastuzumab · 511 keV photon pair

How it works

Radiolabelled HER2 binder; whole-body receptor map.

Strengths
  • Captures inter-lesion heterogeneity
  • Non-invasive re-assessment
Limitations
  • Not approved; small trials
  • Antibody tracers need days
