Needle guides that let a prostate biopsy be taken through the skin behind the scrotum under local anaesthetic, instead of through the back passage. NICE recommends them as an option.
NICE HealthTech guidance HTG680 (formerly DG54, published 1 June 2023) recommends "Local anaesthetic transperineal (LATP) prostate biopsy using the freehand needle positioning device PrecisionPoint" as an option for diagnosing prostate cancer. It adds that three other freehand devices, EZU-PA3U, Trinity Perine Grid and UA1232 puncture attachment, "are expected to have similar cancer detection rates and adverse events to those of PrecisionPoint" and are also recommended, though with less evidence. It found "not enough evidence to recommend double freehand LATP prostate biopsy using the CamPROBE device", and encourages centres to join the TRANSLATE trial comparing transrectal with LATP biopsy. A guide attached to the transrectal ultrasound probe directs a coaxial needle through the perineal skin into the prostate, so many cores can be taken through one or two skin punctures under local anaesthetic.
