Alliance A021806 is the trial the field is waiting for: 358 people with operable pancreatic cancer randomised to surgery first or chemotherapy first with the same total course of modified FOLFIRINOX. It has finished recruiting; the registry gives December 2028 for the primary completion and no results were published in an indexed journal by September 2026.
A021806 randomises patients with resectable pancreatic ductal adenocarcinoma (stage I to III by AJCC 8th edition, adenosquamous included) to perioperative modified FOLFIRINOX (eight cycles, resection, four cycles) or adjuvant modified FOLFIRINOX (resection, twelve cycles), the regimen and duration taken from PRODIGE 24; the primary endpoint is overall survival by intention to treat and the design paper (Annals of Surgical Oncology 2024) sets out the resectability criteria, central radiology review and quality-of-life questionnaires that answer the problems seen in SWOG S1505. ClinicalTrials.gov on 24 September 2026 lists the trial as active but no longer recruiting with 358 participants (actual), a start date of 13 October 2020, primary completion on 31 December 2028 and study completion in November 2030, at sites in the United States and Canada only. No primary publication was found in Europe PMC on that date, so the corpus records the design and status without results; together with the Dutch PREOPANC-3 (perioperative against adjuvant FOLFIRINOX, 378 patients, Netherlands and Sweden) it is the phase 3 evidence that will decide whether NICE and other guidelines move neoadjuvant chemotherapy for resectable disease out of trials.
