PREOPANC-3 is the European twin of Alliance A021806: the same modified FOLFIRINOX given either around surgery or only afterwards for operable pancreatic cancer. Recruitment of 378 patients has closed and the first results are expected after the January 2027 primary completion date.
PREOPANC-3 (protocol in BMC Cancer 2023) randomises patients with resectable pancreatic cancer to perioperative or adjuvant modified FOLFIRINOX with overall survival as the primary endpoint; ClinicalTrials.gov on 24 September 2026 lists it as active but no longer recruiting, 378 participants (estimated), started 7 September 2021, primary completion January 2027 and completion July 2029, at sites in the Netherlands and Sweden. It follows PREOPANC-1 (neoadjuvant chemoradiotherapy beat upfront surgery at five years) and PREOPANC-2 (neoadjuvant FOLFIRINOX equal to neoadjuvant chemoradiotherapy), and with A021806 it will answer the surgery-first question that NORPACT-1 answered against neoadjuvant treatment in a smaller Nordic trial.
