BARBICAN, run from Barts in London, tests whether adding the AKT-blocking tablet ipatasertib to pre-operative chemotherapy and atezolizumab makes more triple-negative tumours disappear. It has finished recruiting 146 women and is due to report in 2026.
BARBICAN (NCT05498896) is a randomised open-label phase 2 trial in 146 women with triple-negative breast cancer, sponsored by Queen Mary University of London with the West German Study Group, MedSIR and Roche, comparing neoadjuvant chemotherapy (paclitaxel, then doxorubicin and cyclophosphamide) with atezolizumab, with or without ipatasertib; the primary endpoint is pathological complete response. It opened in December 2018 and the registry lists primary completion in August 2026 with the trial active, not recruiting. Its context is the failure of AKT inhibition in metastatic triple-negative disease (IPATunity130 for ipatasertib, CAPItello-290 for capivasertib) and of atezolizumab in early disease (NeoTRIP, GeparDouze, ALEXANDRA), so a positive result would need confirmation.
