NeoTRIP added the immunotherapy atezolizumab to platinum chemotherapy before surgery without anthracyclines and did not significantly increase the share of women whose tumour disappeared (49 versus 44 percent); its main endpoint, whether fewer women relapse, is still awaited.
NeoTRIP (NCT02620280) randomised 280 patients (278 in the registry) with triple-negative early high-risk or locally advanced breast cancer to neoadjuvant carboplatin AUC 2 and nab-paclitaxel 125 mg/m2 on days 1 and 8 every 3 weeks for eight cycles with or without atezolizumab 1,200 mg, followed by four cycles of adjuvant anthracycline. The primary endpoint is event-free survival; pathological complete response, a key secondary endpoint, was 48.6 percent with atezolizumab and 44.4 percent without (odds ratio 1.18, 0.74 to 1.89, p 0.48). Serious adverse events and liver transaminase abnormalities were more frequent with atezolizumab. PD-L1 expression was the strongest predictor of pathological complete response (odds ratio 2.08). The trial is unusual in omitting anthracyclines from the neoadjuvant phase, which is one explanation offered for the smaller immunotherapy effect than in KEYNOTE-522 and IMpassion031; event-free survival follow-up continues and no primary result was indexed in September 2026.
