IO Biotech makes an off-the-shelf cancer vaccine that trains T cells to attack both tumour cells and the immune-suppressing cells around them. Its large melanoma trial narrowly missed its main goal in 2025 and the company is now reviewing its options.
Overview
IO Biotech (Nasdaq: IOBT), headquartered in Copenhagen with a US office in New York, develops immune-modulatory cancer vaccines on its T-win platform. Lead candidate Cylembio (IO102-IO103) stimulates T cells against indoleamine 2,3-dioxygenase 1 (IDO1) positive and PD-L1 positive cells, aiming to kill both tumour cells and immune-suppressive cells in the tumour microenvironment. In the phase 3 IOB-013 trial in first-line advanced melanoma, Cylembio plus pembrolizumab gave median progression-free survival of 19.4 months versus 11.0 months for pembrolizumab alone (hazard ratio 0.77, 95 percent CI 0.58 to 1.00, p=0.0558), narrowly missing the prespecified significance threshold of p≤0.045 (ESMO, October 2025). Five-year outcomes from the phase 1/2 trial were published in Nature Communications in December 2025. In January 2026 the company engaged Raymond James to explore strategic alternatives and announced a workforce reduction to preserve cash.
