A method that reads DNA methylation in blood to measure metastatic breast cancer and call its oestrogen receptor status without tumour tissue.
METER exploits tumour-type-specific DNA methylation patterns for ctDNA detection, quantification and subtyping from plasma low-pass (0.5 to 1x) whole-methylome sequencing. In longitudinal samples from metastatic breast cancer it showed a stronger association with clinical outcomes than other ctDNA methods and matched circulating tumour cell counts, even at tumour content below 3 per cent. In the full text, oestrogen receptor calls in samples with at least 5 per cent tumour content reached an accuracy of 0.93 (95% CI 0.85 to 0.97) when the MIMESIS-1 and MIMESIS-2 cohorts were combined.
Methylation in blood can say whether advanced breast cancer still carries the oestrogen receptor. Subtyping still needs percent-level tumour content.
