Noni is a Polynesian fruit sold worldwide as a juice and supplement, often with cancer claims. There are no trials in cancer, and case reports link noni juice to liver injury, including one liver failure needing a transplant, dangerously high potassium, and blocking the effect of warfarin.
Noni (Morinda citrifolia) is used in Polynesian traditional medicine and marketed globally as fresh or fermented juice and as capsules. Memorial Sloan Kettering's About Herbs summary describes antitumour, immune and antioxidant effects in preclinical work and small clinical studies only for postoperative nausea, smokers' lipids and hangover symptoms; none tested it against cancer. MSK lists case reports of hepatotoxicity with noni juice products, including acute liver failure needing transplantation in a 56-year-old woman who drank noni with other products, hyperkalaemia from its high potassium content, resistance to warfarin, and loss of seizure control from low phenytoin levels. It advises caution in kidney insufficiency and diabetes, and warns that its antioxidant effects may reduce the activity of some chemotherapy or radiotherapy, that it adds to potassium-sparing diuretics and ACE inhibitors, and that it modulates UGT enzymes in vitro.
Iridoids, anthraquinones and polysaccharides with antioxidant and immune effects in laboratory models; the anthraquinones are the suspected cause of liver injury.
