AVAglio
AVAglio tested adding the anti-blood-vessel drug bevacizumab to standard radiotherapy and chemotherapy for newly diagnosed glioblastoma; the tumours took longer to grow on scans, but people did not live any longer, so bevacizumab is not used up front for this cancer.
Overview
AVAglio was a double-blind placebo-controlled phase 3 trial in newly diagnosed glioblastoma. After surgery or biopsy, patients received radiotherapy with concurrent and adjuvant temozolomide and were randomised to bevacizumab or placebo throughout, followed by single-agent maintenance.
Bevacizumab prolonged progression-free survival, one of the two primary endpoints, but did not improve overall survival, the other. The parallel US trial RTOG 0825 reached the same conclusion, and together they closed the question of first-line bevacizumab in glioblastoma; the drug remains in use for symptomatic relief and at recurrence, where its 2009 accelerated approval was granted.
