INTELLANCE-1 tested an antibody-drug conjugate that targets the EGFR amplification found in about half of glioblastomas, added to standard treatment; it did not help people live longer, and the drug was abandoned.
Overview
Depatuxizumab mafodotin (ABT-414) is an antibody against a conformation of EGFR exposed on amplified or mutant tumour cells, linked to the microtubule toxin monomethyl auristatin F. INTELLANCE-1 randomised 691 patients with newly diagnosed glioblastoma and EGFR amplification to standard radiotherapy and temozolomide with depatuxizumab mafodotin or placebo. The primary endpoint was overall survival.
An interim analysis found no overall survival benefit and the trial was stopped for futility, although progression-free survival had favoured the drug. Corneal toxicity affected most treated patients. The programme was discontinued, and glioblastoma remains without an effective targeted therapy despite EGFR alterations in half of tumours.
