HERBY
HERBY asked whether adding the anti-blood-vessel antibody bevacizumab to radiotherapy and temozolomide would help children with high-grade brain tumours; it did not delay the tumours returning, so the drug is not part of standard treatment for these children.
Overview
HERBY was an open-label randomised phase 2 trial in children and adolescents with newly diagnosed high-grade glioma outside the brainstem. After surgery, patients received radiotherapy with concurrent and adjuvant temozolomide, with or without bevacizumab.
Adding bevacizumab did not improve event-free survival, the primary endpoint, as judged by central review, and did not improve overall survival. The trial did produce one of the best-characterised molecular cohorts of paediatric high-grade glioma; its biology sub-study showed that histone H3 and IDH mutation status, rather than histology, separated the outcomes, which fed into the WHO classification of these tumours.
