ECLIPSE was the randomised test of the Johns Hopkins pancreatic cancer vaccine strategy, a whole-cell GVAX vaccine followed by a Listeria vector carrying mesothelin; after promising early results it did not beat chemotherapy, with survival of 3.7 months against 4.6, and the programme ended.
ECLIPSE randomised 303 patients to cyclophosphamide with GVAX pancreas followed by CRS-207 (arm A), CRS-207 alone (arm B) or physician's choice single-agent chemotherapy (arm C). In the primary cohort of 213 patients who had failed two or more prior lines including gemcitabine, median overall survival was 3.7 months (95 percent confidence interval 2.9 to 5.3), 5.4 months (4.2 to 6.4) and 4.6 months (4.2 to 5.7) in arms A, B and C, with no difference between the vaccine combination and chemotherapy (hazard ratio 1.17, 0.84 to 1.64). Chills, pyrexia, fatigue and nausea were the commonest adverse events and there were no treatment-related deaths. The earlier phase 2 had reported 6.1 months with the combination against historical controls, a reminder that single-arm vaccine results in this disease have repeatedly failed to hold up when randomised, as ELI-002 7P did again in AMPLIFY-7P (2026); the personalised mRNA approach (autogene cevumeran) is now the vaccine strategy in randomised testing.
