JCOG0403 gave the first prospective evidence that stereotactic lung radiotherapy works in patients who could have had surgery, not only in those too frail for it, and set the 48 Gy in four fractions schedule used across Japan.
Overview
JCOG0403 was a two-cohort phase 2 trial of stereotactic body radiotherapy (48 Gy in four fractions) for stage IA non-small-cell lung cancer, enrolling 65 operable and 104 inoperable patients. Three-year overall survival was 76.5 percent in the operable cohort and 59.9 percent in the inoperable one, with acceptable toxicity (Nagata and colleagues, International Journal of Radiation Oncology Biology Physics 2015). It prompted the randomised comparisons of surgery and stereotactic radiotherapy that followed.
